This form is specific to individuals looking to purchase a puppy or adult dog for the prospective purpose of Service Dog work. By filling out this form you acknowledge that you are aware of current ADA law in regards to service dogs, as well as any local laws that pertain to the training/handling/and public access of these dogs.
